Every year, we ask families, teachers, and staff of grades 3-K through 12, and students in grades 6-12 to take the NYC School Survey. The survey gives school leaders a better understanding of what members of their school community think about their school. The information collected by the survey is designed to support a dialogue among all members of the school community about how to make the school a better place to learn. Visit https://surveys.panoramaed.com/nycdoe/login to complete the survey.

Dear Parent/Guardian,
Each spring, students take the English Language Arts (ELA) and Mathematics tests in grades 3-8 and the Science tests in grades 5 and 8. These tests are used to measure how well students perform on
New York’s Next Generation Learning Standards. Over the past few years, schools across New York City have begun to administer these state tests
on a computer. All Grade 3, 4 and 5 students will take the New York State ELA, Math and Science exams on a computer.
Please note the following:
â—Ź Practice: Before the test is given, your child will have a chance to practice taking the test on a computer while in school. This will help your child become familiar with the online
testing format. You and your child can review and practice using the online Question Sampler (https://ny.nextera.questarai.com/tds/#practice)
â—Ź Testing schedule: Your child will have as much time as your child needs, within the limit of the school day, to complete the test.
Assessment Content Area Grades Administration Date(s)

Dear Parents and Guardians,
As we begin another exciting school year, I want to take a moment to say welcome back. We’ve officially kicked off the year, and I’m happy to report that the first day of school went smoothly. It was a joy to see our hallways filled with energy, enthusiasm, and eager learners. Thank you for helping make that possible with your support, preparation, and positive mindset.
To our returning families, thank you for your ongoing trust and support. Your involvement in your child’s learning makes a meaningful impact every day.
To those new to our school community, welcome! We’re so glad you’re here. Your fresh perspectives, ideas, and questions will help us grow even stronger. We hope you already feel seen, valued, and supported.
This year, we’ll continue our focus on academic excellence, leadership development, and fostering a school culture where every student feels safe, challenged, and celebrated. We believe that when schools and families work together, students thrive and that belief is at the heart of everything we do.
Here are a few reminders and updates as we move forward:
On a personal note, as I begin my 12th year serving as principal, I continue to be inspired by the strength and heart of this school community. I look forward to learning from your insights, collaborating through our challenges, and celebrating our shared successes in the months ahead. This year will undoubtedly bring both opportunities and challenges but together, we are builders of possibility and champions for children. Let’s make it a year filled with purpose, joy, and growth.
Thank you again, and welcome to the 2025–2026 school year!
In Partnership,
Elizabeth Mitchell
Principal
PS 64Q Distraction-Free Policy for Personal Electronic Devices 2025-2026
At PS 64Q, we are committed to fostering a safe, respectful, and focused learning environment for our students. In alignment with New York State Distraction-Free Schools Policy, New York City Public Schools regulations, and in collaboration with the PS 64Q School Leadership Team, the following policy regarding personal electronic devices is in effect.
An "internet-enabled electronic device" is defined as any device capable of connecting to the internet and enabling the user to access online content. This includes, but is not limited to, cell phones, smartphones, wireless headphones, smartwatches, laptops, tablets, iPads, and portable music or entertainment systems.
Students are not permitted to use or access their personal internet-enabled electronic devices from the moment they enter the school building, at 8:00am, until the last class of the day ends, at 2:20pm, from bell to bell, including during breakfast, lunch, and recess. Additionally, students are not permitted to use their cellphones or other electronic devices in the bathroom at any time. During the school day, students may use school-issued or NYCPS-issued devices for educational purposes as directed by staff.
If a child is picked up late, they cannot use their device while waiting for their parent or guardian. If a student attends the DYCD after-school or a PS 64Q after school program, their device must remain off and locked in the designated pouch for the duration of the program.
âž” Upon arrival, students must power off their devices.
â—† Students will place their devices in a Faraday pouch and secure it in the presence of school staff.
â—† Students will store their locked pouches in their backpacks for the day.
â—† At the end of the school day (2:15pm), students will unlock their pouches in their classroom.
â—† Students with approved early dismissal will be able to unlock their pouch in the main office.
âž” In case of emergency or urgent circumstances, parents or guardians can call the main office at
718-845-8290, to reach their child.
âž” In case of emergency or urgent circumstances, students may access phones in the main office to reach their parents or guardians.
âž” In case of emergency or urgent circumstances, the school will use Class DOJO and NYC Public Schools DOE Messaging System to communicate information to parents or guardians. To receive automated messages from the school, parents and guardians must have a NYC Schools Account.
◆ To create an account, visit https://schoolsaccount.nyc and click on the “Create Account” link
on the login page. You will need a creation code, which can be obtained from the Parent
Coordinator, Jeiry Reyes.
âž” Students are allowed to use their device if they have an individualized education program (IEP) or 504 Plan that includes use of an internet-enabled device and do not have a DOE-issued device for such purpose.
âž” Parents/guardians must contact Elizabeth Mitchell, Principal, at 718-845-8290, if a student
requires an exception for reasons such as: medical monitoring/treatment (for example, to monitor blood sugar or other similar circumstances).
âž” Exceptions will be processed and approved within five business days.
âž” Use of personal electronic devices in violation of this school policy, the NYCPS Discipline Code,
Chancellor’s Regulation A-413, and/or the NYCPS Internet Acceptable Use and Safety Policy will have their devices confiscated and held by an administrator. Confiscated devices will only be returned to the student’s parent or guardian. Students will be subject to progressive discipline, meaning that consequences will increase in severity based on the nature and frequency of the violation.
âž” The school is not responsible for any damaged, lost, or stolen electronic devices.
➔ In the unlikely event that an electronic device is stolen or damaged at school, parents can submit a claim to the Comptroller’s Office. More information on submitting a claim is available on the
➔Families will incur a replacement fee for lost or damaged storage devices in the amount of $5.00.
Please note this Cellphone and Electronic Device policy is available on the school website http://www.ps64q.org/ and will be shared with all families and staff at the start of the school year.

How do I create an NYC Schools Account?
There are three ways you can create a NYCSA account. You can either visit your child’s school in-person to receive a temporary password, contact your school and verify your identity via teleconference and receive a temporary password, or you can use the Account Creation Code on the letter sent home with your child.
In Person or Online
1. Set up an appointment with your child’s school.
2. Provide a valid photo ID (for example, your passport, your drivers’ license, IDNYC), your email address, and your child’s 9-digit student ID number, which can be found on his or her report card. If via teleconference, the school will verify the identity of the parent/guardian or eligible student by voice or sight.
3. When you create the account at your school or by teleconference, you’ll receive a receipt with your temporary password printed on it either in-person or by email.
4. Go to https://schoolsaccount.nyc and enter your email address and temporary password. Please note that the temporary password expires in 30 days. You will be asked to create a new password for your account, set up your security questions, and confirm your email address.
Self-Serve
You will need the Account Creation Code that was sent home with your child in a letter. You can also request a letter from your child’s school. You will need this code and the child’s 9-digit student ID number to create an NYC Schools Account or to add additional children to an existing account.
1. Visit https://schoolsaccount.nyc and select the “Create Account” link found on the login page. |2
2. Enter your name, email address, and cell phone number to begin receiving notifications. You can create a basic account without an account creation code, but to link your student to the account, you will need the code.
3. Select “Validate Email” after you have entered your contact information. Keep this page open.
4. Open your email to get the validation number. Enter the number into the “Validate Email” page.
5. Proceed to create your password.
6. Proceed to create your account security questions.
7. Enter the unique Account Creation Code, which is specific to you and your student.
What Information Will I Have Access To?
Each adult associated with your child's record is assigned the role of either a Custodial, Non-custodial, or Additional user. These roles determine how much of your child’s information they can see in MyStudent. All user roles have access to Parent University and the Bullying Portal.
• As a Custodial user, you can see all information available in your child’s NYC Schools Account. Also, you can invite other adults to view your child's information. In addition, custodial users can update the emergency contact information of adults on the student’s record in the application to be contacted directly about emergencies.
• As a Non-custodial user, you can see most information in your child’s NYC Schools Account. Noncustodial users can also update their cell phone numbers in the application in cases of emergencies.
• As an Additional user, you can see information the child’s custodial parent shares with you. Where can I find help on NYC Schools Account? If you need assistance or have questions about navigating your account, contact your child’s school or call 311. You can also learn more by visiting the NYC Schools Account webpage at https://www.schools.nyc.gov/nycsa.
How do I update my child’s emergency contact information?
If you are a parent who lives with your child, or a custodial guardian, you can update your own home, cell, and work phone numbers and email address of yourself or other additional adults on your child’s record in your NYC Schools Account, using the Emergency Contact page. You also have the option to update your preferred written or spoken language. You can also update your child’s health information to share with your child’s school. This includes your child’s physician name and phone number, any known diagnoses, allergies, limitations that may limit physical activity, and health insurance status. If you need to update your home address, update siblings on your child’s record, or update order of protection information, you must contact your school directly to make the change(s).
How do I find more information about the Bullying Portal? If you need assistance with using the Bullying Portal or additional resources to help stop discrimination, visit the Respect for All: Fostering Anti-Bullying Practices webpage: https://www.schools.nyc.gov/school-life/schoolenvironment/respect-for-all.
