Back To School Information
Good Morning, Families! ☀️
We hope you are enjoying the last few weeks of summer! As we prepare for Back to School, here are a few important reminders.
📁 Please check your child’s end-of-year folder.
At the end of the school year, each student received a folder containing:
• Final Report Card with next year’s class number (example: 101, 203)
• i-Ready Math and ELA data
• School Supply List
• School Calendar
• Blue Card
• Important information about our reinstated school uniform policy
Please take some time to review all of these items before school begins.
🔵 BLUE CARD
Please complete the Blue Card and return it on the first day of school.
🏥 HEALTH FORMS & MEDICATION
If your child has a medical condition that requires medication or special accommodations at school, such as asthma, please visit your child’s doctor before school begins.
Your child may need updated forms, such as a Medication Administration Form (MAF) or 504 Accommodation Plan.

💉 IMMUNIZATIONS
Please check with your child’s doctor to make sure all required immunizations are up to date.
