π Please check your childβs end-of-year folder.
At the end of the school year, each student received a folder containing:
β’ Final Report Card with next yearβs class number (example: 101, 203)
β’ i-Ready Math and ELA data
β’ School Supply List
β’ School Calendar
β’ Blue Card
β’ Important information about our reinstated school uniform policy
Please take some time to review all of these items before school begins.

Please complete the Blue Card and return it on the first day of school.
π₯ HEALTH FORMS & MEDICATION
If your child has a medical condition that requires medication or special accommodations at school, such as asthma, please visit your childβs doctor before school begins.
Your child may need updated forms, such as a Medication Administration Form (MAF) or 504 Accommodation Plan.

Dear Parents of Fourth Graders:
The following is a list of supplies your child will need for Fourth Grade
Please bring the following items on the 1st day of school (9/5/2024)
β 4 marble notebooks (no spiral notebooks)
β 6 plastic coated folders (yellow, red, green, purple, blue & orange)
β 3 dozen sharpened pencils
β 3 packages of dry erase markers
β 1 dry erase eraser
β 1 soft pencil case (no pencil boxes)
β 2 package of glue sticks
β 1 pair of scissors
β 1 package of 24 count crayons
β 1 package of colored pencils
β 2 sets of earbuds (no headphones, they are too bulky)
β 2 packages of post-it notes
β highlighters (blue, yellow & pink)
Please bring the following items on the 2nd day of school
β 1 box gallon size ziploc bags
β 2 boxes of tissues
β 1 roll of paper towels
β Boys bring 1 package of baby wipes
β Girls: bring 1 hand sanitizer
